eForm (staff only)
- Login to University Portal.
- Click on the 'Banner Services' tab and select 'ITSC eForm'.
- Move to 'Multimedia & AV Support Services' box and select 'On-loan and Support Services'.
- Follow the instructions and complete the eForm.
