Application for Registry Services


Application forms, together with general information and guidelines, are available in Word 7.0 format (Chinese version) and PDF format.

Click the type of form to download the document. Duly completed application forms should be returned to the Registry in person/by fax/by post. (Note: For all applications by fax/post, a copy of the applicant's personal ID must be attached.)
